Approximately 10 words that will catch the attention of summit participants and accurately describe your session.

*Forum Description
50 to 75 words used in the Summit program to succinctly, persuasively, and accurately tell Summit participants what to expect from the session. The description should be completed in such a way to help Summit participants determine the scope and level of the session and its appropriateness to their needs. Session descriptions should be modeled after the samples and should answer the following two questions: “What will participants in this session learn?" and "Who will this session particularly benefit?"

*Narrative
In 250 to 500 words, (a) provide a brief description of the topic, initiative, program, or practice that will be the focus of the forum, including evidence of – or a plan to evaluate – its effectiveness; (b) list the intended outcomes for participants in the forum; (c) list and briefly describe activities planned for the forum. The forum activities should be designed to help ensure achievement of the forum outcomes. Part or all of the forum plan may be included in the Summit program.
